2.  Parent/Guardian Consent Form

3.  Pastoral Reference Form -  This form will be sent to your pastor from MO YOM Staff to be completed by your pastor and returned.  

Once all of these steps have been completed, your application is considered submitted and we will begin the review process.  

4.  Upload Picture of Passport - Please upload a picture of the name page of your passport.  Make sure that all information on the page can be read with no glares.
